2025年VBS报名 – VBS Registration

我准许纽奥良华人浸信会及其暑期圣经班的同工在我的孩子受伤、生病、或有其它健康问题时采取必要的医疗处理， 直到联络到我本人为止。我签署此表豁免教会的同工在此类紧急处理期间产生的任何财务责任。 此豁免仅在2025年7月18至20的暑期圣经班期间有效。

I give permission to the Chinese Baptist Church of New Orleans (NOCBC) and its Summer Bible School staff to take necessary medical care for my child(ren) in the event of accident, injury, ill, or have other health problems, until I can be contacted. I sign this form to release the staff of the Church from any financial liability incurred during this emergency. This waiver is valid only during Summer Bible School, July 14-16， 2025.

此外，我同意纽奥良华人浸信在暑期圣经班期间对我的孩子进行拍照和录像，并理解照片/视频可能会被纽奥良华人浸信会 用于剪贴簿、视频演示、宣传等目的。Additionally, I consent to Chinese Baptist Church of New Orleans taking photos and videos of my child during this Vacation Bible School with the understanding that the photos/videos may be used by Chinese Baptist Church of New Orleans for scrapbooking, video presentations, promotional purposes, etc.

我自愿免除新奥尔良华人浸信会 (NOCBC) 与我的孩子参加这个假期圣经学校有关的任何及所有责任、索赔、诉讼或诉讼权利。  I voluntarily release the New Orleans Chinese Baptist Church (NOCBC) from any and all liability, claim, suit, or right of action in connection with my child's attendance at this Vacation Bible School.
